Indoor Soccer - Mail In Registration Form

ALL COMMUNICATION IS DONE BY EMAIL.  IF YOU HAVE NOT HEARD FROM US -
IT IS LIKELY BECAUSE WE CAN'T READ YOUR EMAIL ADDRESS. 
All games are played on Saturdays starting October 16th and schedules will be sent out after registration closes.

ALL PLAYERS MUST HAVE  VALID COMMUNITY ASSOCIATION MEMBERSHIP.  IF YOU DON'T HAVE A MEMBERSHIP, YOU CAN OBTAIN ONE FROM YOUR COMMUNITY ASSOCIATION OR FROM THE SOUTH ZONE.  IF YOU WANT TO PURCHASE ONE FROM THE SOUTH ZONE, PLEASE INCLUDE $5.00 WITH YOUR REGISTRATION FEE. 
NOTE:  PLAYERS CANNOT BE REGISTERED WITHOUT HOLDING A VALID COMMUNITY MEMBERSHIP

Waiver - By participating in any South Zone Recreation Board program the participant, volunteer or other  assumes all responsibilities for injuries, lost or stolen items, damages, claims, demands, lawsuits, judgments, costs or other, and releases the South Zone Recreation Board from any  liability.  All  program participants under the age of 18 years must  be accompanied by an adult when attending any South Zone Program.  Thank you.
